The Blizzard Launcher includes a piece of software called the Warden which will actually scan your computer in an effort to detect botting software. All it would take is someone to raise a suspicion and they could look over those scans to see if there is anything running

if someone suspects that you are using something, they can message a GM and if they message you and you dont reply they can assume you are using a bot and warn you, suspend you for a while, or ban you.

Everyone I know who has used one has first gotten a warning then got banned. WoW is practically a license to print money for Blizz and they don’t want cheaters ruining the experience for legitimate gamers. On a personal note, how messed up is it to cheat in an MMO, just play the game.
